There is risk associated with all treatments and exercises provided by ReConnect Health Centre & Physiotherapy PC Inc, and such risks to the individual receiving treatment may include, but are not limited to, personal injury up to and including death, illness, damages and other losses.  An individual that participates in any treatment or exercise provided at ReConnect Health Centre & Physiotherapy PC In, be it in person or online, voluntarily puts his or her own health and safety at risk.  By participating in any treatment or exercise by ReConnect Health Centre & Physiotherapy PC Inc the individual demonstrates that he or she accepts and fully assumes all risks and hazards associated with his or her participation in treatment or exercises by ReConnect Health Centre & Physiotherapy PC Inc, including but not limited to personal injury up to and including death.

Furthermore, by participating in any treatment or exercises provided by ReConnect Health Centre & Physiotherapy PC Inc the individual RELEASES ReConnect Health Centre & Physiotherapy PC Inc  and its owners, directors, officers, physiotherapists, employees, agents, and representatives ReConnect Health Centre & Physiotherapy PC Inc  of and from any and all liability and responsibility for all damages, claims, or causes of action that  the individual or his or her respective heirs, executors, administrators, or assigns may have, for any loss, illness, personal injury including death or damages due to any cause whatsoever, including negligence, gross negligence, breach of contract, or breach of any statutory or other duty of care, on the part of ReConnect Health Centre & Physiotherapy PC Inc .  Additionally, by participating in any treatment or exercises provided by ReConnect Health Centre & Physiotherapy PC Inc. the individual or his or her respective heirs, executors, administrators, or assigns, WAIVES any legal rights he or she may have against ReConnect Health Centre & Physiotherapy PC Inc. and its directors, officers, physiotherapists, employees, agents, or representatives including but not limited to the right to pursue a legal action for personal injury including death.

An emergency situation may arise prior to, during or after treatment or exercise with ReConnect Health Centre & Physiotherapy PC Inc. By participating in treatment or exercise, the individual agrees that they will contact emergency care to be administered in the event of an emergency. 

Agreement to the foregoing is a condition precedent to participating in any and all treatments and exercise offered by ReConnect Health Centre & PC Inc.  By participating in any treatment and exercise by ReConnect Health Centre & Physiotherapy PC Inc. the individual clearly and irrevocably agrees to the foregoing.
